What is a gummy smile? Causes and assessment

Learn about possible reasons gums show when smiling and individual assessment.

What is a gummy smile?

It is natural for the upper gums to show when smiling. Some people may feel their gums are more visible than they would like. This appearance alone does not mean there is a disease; assessment is based on the person’s oral health, anatomy and wishes.

What can cause the appearance?

The position of the gum on the tooth, the visible length of the teeth, movement of the upper lip and the structure of the upper jaw can affect appearance. More than one factor may be present. Gum enlargement or inflammation is also assessed separately. For this reason, the cause or appropriate treatment cannot be determined from a photograph alone.

What is assessed during an examination?

The dentist may examine lip position while smiling and at rest, as well as the teeth and gums. Assessment of gum health is an important step. Imaging or opinions from other specialties may be requested when needed. It may not be appropriate to apply the same measures or method to everyone.

How are treatment options determined?

The need for treatment and possible methods depend on the reason for the appearance and the person’s expectations. In some cases monitoring alone may be sufficient; in others, gum or orthodontic assessment may be considered. Benefits, limitations and risks should be discussed individually. Existing gum disease may need assessment first.

How can you prepare for the consultation?

Describe what concerns you in your own words and share your expectations. Ask the dentist what the cause might be, why a method is recommended and what alternatives exist. A healthy mouth and the individual’s preferences are important parts of an aesthetic decision; a particular appearance or result cannot be guaranteed.

Why is gum health assessed?

Signs such as redness, swelling or bleeding should be assessed as health concerns separately from the appearance of a smile. If a condition related to gum disease is present, a care plan may be recommended. Wanting a procedure for appearance does not make assessment of underlying health problems unnecessary. The dentist reviews the findings and available options with you.

Treatment and follow-up decisions are considered with your dentist in light of individual findings; general information does not replace an examination.
